Families often tell us they worry when a loved one is becoming isolated or no longer participating in activities they once enjoyed. Social Support can provide reassurance that someone is helping your loved one stay connected, engaged, and involved in their community.

Social Support may be available through Support at Home, CHSP, Veterans’ Home Care, or Private Home Care, depending on your eligibility, support plan, and individual circumstances.
